PIAA Transfer Form

Steps

1. Family notifies receiving school Athletics Office
2. Athletics office completes Section 1 of the PIAA Transfer Form 
3. Family completes and signs sections 2 and 3
4. SCASD Principal completes and signs Section 4 of the PIAA Transfer Form
5. Athletics Office submits form to the PIAA Portal

 

Postseason Waiver Form

Steps

1. Athletics Office completes Section 1
2. Family completes, signs, and returns Sections 2, 3, & 4. 
3. SCASD Principal completes and signs document 
4. Athletics Office submits form to the PIAA Portal

Foreign Exchange Student-Athlete Form

Steps

1. The host family and student athletes complete, signs, and returns the above form to the Athletics Office.
2. Athletics Office submits form to the PIAA Portal
