For driving your own child:
 
Parents who plan transporting their athletes to or from the travel meets must fill out and sign this form: 
 
 
 Please sign and email it to both SCASD Athletic Office and Head Coach prior to the day of the meet.
You can fill out one form for the entire season by indicating all the away meet dates on the form.
Filling out the form does not prevent your athletes from riding a bus with the team, but rather gives you an option to drive them if needed.
 
Note: if you drive your athlete to the meet, they can't ride the team bus on the way back; but if athletes ride the team bus to the meet, parents can drive them back (signed form is required). 
 
Athletes must check out with the Head Coach before leaving the meet with a parent!

If you are driving another athlete who is not part of your family, please fill out this form below and email it to both SCASD Athletic Office and Head Coach prior to the day of the meet.
